- It’s five years after Drishyam 2. Vijay (Georgekutty) is settled — his life story has even been turned into a successful film, which he insists isn’t based on real events, despite reporters digging into it.
- Jose, a man Vijay once helped, reappears with his wife to thank him — a small subplot about loyalty and gratitude that runs alongside the main story.
- At home, Vijay and Nandini (Rani) are trying to arrange Anju’s marriage. Every promising match mysteriously falls through: someone is sabotaging it from the shadows.
- Vijay traces the interference to a man named Rajan — a former policeman who was suspended years ago for helping Vijay, and whose life fell apart afterward (his daughter attempted suicide and lost her ability to speak). Rajan blames Vijay for abandoning him and wants revenge.
- Separately, Vijay notices a freshly dug and refilled pit on his land. He’s being secretly watched by a man called Sahadevan — a suspended ex-cop, now unstable and broke, who’s quietly building a case against Vijay with help from his journalist daughter.
- Sahadevan illegally gets hold of Anju’s psychiatric records and starts pressuring people connected to the original case. His activities reach the police chief, who orders his arrest.
- Here’s the real plot behind it all: Mahesh Deshmukh (Prabhakar) — father of the boy who died in the very first film — approaches the police. His wife Meera (Geetha), the IG from the earlier films, is living abroad and has sunk into severe depression over their son’s death and Vijay’s continued freedom. Out of love for her and resentment toward Vijay, Mahesh decides to act.
- Mahesh has secretly hired Sahadevan and quietly engineers a marriage proposal for Anju with a man named Avira — manipulating him into the plan by solving his financial troubles.
- The real trap: Mahesh and his police contact plan to stage an incident where Anju is drugged and falsely blamed for Avira’s death, backed by fabricated medical records meant to link her to violent behaviour — and tie the whole thing back to the first killing.
- The plan goes wrong. Sahadevan breaks from the script, Anju unexpectedly wakes up mid-plan, panic breaks out, and Sahadevan injures Anju before fleeing — with Avira nowhere to be found.
- Vijay arrives, works out what the dug-up pit was really for, rallies help through his brother-in-law, and tracks Sahadevan down. Sahadevan is caught and forced to confess — implicating Mahesh and the police officer backing him.
- Vijay then offers to surrender to the police — but on his own conditions, tied to his family’s safety, with his lawyer by his side.
- One final twist surfaces before the credits: Anju’s injury wasn’t actually caused by Sahadevan at all — it was Vijay himself, done deliberately to throw the investigation off track and protect her, with Sahadevan framed for it.

Drishyam: The Conclusion—What’s confirmed to be different in the Hindi version, from the trailer:
- A new character, SP Crime Branch Rajveer Singh Tomar (Jaideep Ahlawat), leads a fresh investigation — he has no Malayalam equivalent.
- Meera (Tabu) is shown saying she’s past wanting justice — she wants revenge, a sharper tone than the Malayalam film.
- A new lawyer, Indrajith Shetty (Prakash Raj), adds a legal-battle layer not present in the original.
A quick note for you: The Hindi version is not a scene-for-scene copy. Director Abhishek Pathak has written a fresh screenplay, and the trailer shows new elements — a new investigating officer and a new lawyer — that don’t exist in the Malayalam film.
